Psychological Evaluation Services and Fee Agreement

This document includes descriptions of evaluation services to help determine what type of testing and assessment would be most useful. Specific tests and measures listed in the packages below are common components offered with the package and are not a complete list of options. Your clinician may choose to include more or fewer assessment measures depending on the goals of testing, and may determine specific instruments are indicated once the interview is complete.  A deposit equal to half of the testing cost is required to schedule all evaluations (with the remaining balance due prior to the first day of testing). Results are typically available about two to three weeks from either the last date of testing or when all rating forms are received, whichever occurs later.  When scheduling, please allow several hours for interview and test administration (this can be broken into multiple sessions if needed).   Your clinician will contact you in advance to help you understand what to expect during your appointment. 


ADHD Evaluation: Ages 13-17
This is a battery of assessments performed when ADHD is suspected, and includes a clinical interview, an abbreviated IQ test, symptom rating scales, and a computerized test of variables of attention.
Assessments: 

  • IQ / Cognitive Assessment (WASI-II) 

  • Clinical interview & Mental Status Exam

  • Parent interview

  • Adaptive Behavioral Scales (ABAS-3)

  • Self-report, Parent & Teacher surveys (Brown EF/A)

  • Continuous Performance Test (TOVA) 

  • Record Review

  • Additional Screening for specific areas if indicated

Estimated time:  

  • 1-2 hours clinical interview 1-2 hours testing, up to 1 hour for document review/collateral interviews, 2-3 hours interpretation and report writing, 1-hour feedback session 

Cost: 

  • $800  (additional fees apply if more time or tests are needed)

Adult ADHD Evaluation: Ages 18+
This is a battery of assessments performed for adults when ADHD is suspected, and includes a clinical interview, a comprehensive emotional and personality assessment, an abbreviated IQ test, symptom rating scales, and a computerized test of variables of attention.
Assessments: 

  • IQ / Cognitive Assessment (WASI-II) 

  • Clinical interview & Mental Status Exam

  • Adaptive Behavioral Scales (ABAS-3)

  • Self-report survey (Brown EF/A)

  • Collateral Interview / surveys if needed 

  • Continuous Performance Test (TOVA) 

  • Personality Inventory (MMPI-3)

  • Substance Use Screening (SASSI-4) (if indicated)

  • Record Review

Adult Mental Health Diagnostic (Abbreviated)
This includes emotional and personality testing, and provides in-depth information regarding an individual’s functioning in both of these areas. This testing helps clarify what a person is thinking and feeling, his underlying personality structure, coping skills and weakness, and why a person behaves as he does. An abbreviated psychological evaluation can be an invaluable resource for providing diagnostic clarification and treatment recommendations.
Assessments: 

  • Clinical interview & Mental Status Exam

  • Personality Inventory (MMPI-3)

  • Substance Use Screening (SASSI-4)

  • Depression and Anxiety screening (if indicated, BDI-2 and BAI)

  • Record Review

  • Additional Screening for specific areas if indicated

Estimated time:  

  • 1-2 hours clinical interview, 1 hour testing, up to 1 hour for document review/collateral interviews, 3-4 hours interpretation and report writing, 1-hour feedback session 

Cost: 

  • $700 (additional fees apply if more time or tests are needed)


Full Adult Mental Health Diagnostic
This includes cognitive, emotional and personality testing, and provides in-depth information on an individual’s functioning in all three of these areas. It’s useful to help understand what a person is thinking and feeling as well as one’s underlying personality structure, cognitive strengths and limitations, coping skills and weaknesses, and motivations behind behavior. In clinically complex cases and/or cases in which it is difficult for someone, to identify the problem or articulate one’s own experiences, a full psychological evaluation can be an invaluable resource, for providing diagnostic clarification and treatment recommendations. 
Assessments: 

  • Clinical interview & Mental Status Exam

  • IQ / Cognitive Assessment (WASI-II) 

  • Adaptive Behavioral Scales (ABAS-3)

  • Personality Inventory (MMPI-3)

  • Substance Use Screening (SASSI-4)

  • Record Review
